Register with Palatine
If you would like to join the patient list at Palatine Group Practice, you will need to fill in a couple of forms. One of them can’t be downloaded, so you will have to get it from the surgery, but the other can be downloaded by clicking here. Alternatively you can collect both from the surgery. When you have completed the forms, you will need to bring them into reception, along with a valid (ie in date) photo ID and a proof of address. As proof of address, we will accept a recent (within 3 months) utility bill, or a bank statement or similar. We cannot accept a mobile phone bill. Examples of ID would be a passport, a driving licence, or similar. If your name has recently changed, eg if you have just got married, but you haven’t yet got your ID changed, please bring your marriage certificate with you. If the photo ID has an address on it, ideally this should be the same as the one at which you are registering, unless there is a clear reason why not. If you are not able to produce either ID or proof of address, please speak to us as it may be possible to make an exception if there are extenuating circumstances.
It has been the policy here to ask for proof of ID and address for around three years. It is important to verify that the patient is who they say they are to ensure that they are entitled to free health treatment. In addition to this, it is common practice for patients to request that doctors sign letters of authority or other legal documents when they have been with the practice for a while, so it is appropriate for us to verify details when the patient joins us.
